The entry should be self-explanatory, except perhaps the fact that by default appears only one field for answers. Such dictatorial trends make little sense, of course - on 'Add Reply' you can add additional input fields.
To demonstrate, we have added one time, admittedly a bit self-referential survey.

I looked at just in your linked post. There are only three possible answers Choice. If I go to edit post, then only three answers are given. The fourth field is empty. Was it because more choice answers? (Your question yes ...)

I think polls already good - in other forums is already a standard.
It is more difficult though, they are creating good content surveys.
