For example, there are free image databases, where you can search for motifs together. The template itself must be drawn in Photosdhop so simple: interior and outer diameters define the area in between with Picture / s and text design.
BTW: On DVDs you should not stick labels. Otherwise you do not have friends that long.

There are a number of printers, not expensive, in which one "photo-printable CDs and DVDs may appeal. The blanks are slightly more expensive than the uncoated, but cheaper than uncoated plus-adhesive labels, which, as Mark writes, anyway are not recommended.
This printer is a small software to create the images attached. You can use the template simply by screenshots as template in Photoshop and invite as transparent layer (12cm x 12 cm, 300 dpi) on the self-made graphics set. The printer software centered Jpeg then finished the car just right. Needless to say, the design with Photoshop as extensive as in "Print Label", the corresponding software of Canon.
